From Referral to Completion
Aftercare Pathway Services LLC follows a consistent process that helps ensure every participant receives appropriate services while keeping referral partners informed throughout the program.
Submit a Referral or Service Request
Requests may be submitted by probation officers, treatment providers, community organizations, employers, educational institutions, healthcare professionals, individuals, or family members.
Referral Review
Information is reviewed to determine program eligibility, service availability, and the most appropriate behavioral programming option.
Participant Enrollment
Participants complete enrollment paperwork, review program expectations, and receive orientation information before services begin.
Program Assignment
Participants are placed into the most appropriate program based on referral information, identified needs, and program availability.
Active Participation
Participants attend structured programming, engage in discussions, complete activities, and develop practical behavioral skills.
Progress Monitoring
Attendance, participation, and progress are documented throughout services. Progress updates may be shared with authorized referral partners when appropriate.
Program Completion
Upon successful completion, participants receive completion documentation and referral partners may receive final reports when applicable.
