Information Review & Initial Contact
Aftercare Pathway Services reviews submitted information and contacts the participant or referral source to discuss program options, availability, and expectations.
Enrollment & Program Agreement
Participants complete required enrollment information, review participation expectations, and receive program instructions before beginning services.
Program Placement
Participants are connected with appropriate programming based on identified needs, goals, referral recommendations, and available services.
Structured Programming
Participants engage in facilitator-led sessions focused on skill development, accountability, problem-solving, decision-making, communication, and practical application.
Progress Tracking & Completion
Attendance, participation, and completion progress are monitored throughout programming. Completion documentation and participation summaries may be provided to authorized referral partners when appropriate.
