I understand that Aftercare Pathway Services LLC provides structured, skills-focused programming designed to support personal development, accountability, decision-making, communication, and positive change.

I understand the purpose and goals of the program.

Participation Agreement

I understand that successful participation requires attendance, engagement, respectful communication, and completion of program activities.

I agree to participate actively and respectfully.

Confidentiality Agreement

I understand that information shared during programming should be respected and kept private. I agree not to share another participant's personal information outside of the program.

I agree to follow confidentiality expectations.

Information Sharing Authorization

I understand that information regarding participation, attendance, and completion may be communicated with authorized referral partners when appropriate and permitted.

I authorize appropriate communication with approved referral partners.

Program Policies

I understand that failure to follow program expectations may impact continued participation. Questions regarding services or expectations may be discussed with Aftercare Pathway Services LLC.
